The effect of soft tissue on the treatment of ankle and pilon fractures

The ankle region is challenging to reconstruct for orthopaedic surgeons owing to its complex bony anatomy and unique functional features. Distal intraarticular tibial fractures named as pilon fractures which mostly result from axial loading has generally complex injury patern and the risk of significant complications. Variables such as fracture pattern, soft-tissue injury, and pre-existing patient factors can lead to unpredictable outcomes. Avoiding complications associated with the soft-tissue envelope is the most important factor to optimize outcomes. In cases with soft tissue problems, the use of temporary external fixation and staged treatment management is helpful in reducing further complications. Preoperative planning and biologically friendly surgical techniques are also critical to prevent complications and increase the likelihood of success.
