Traction is one of the oldest methods for treatment in medical history. Frequently it is used as skeletal or skin traction. Although skeletal traction was historically first used for definitive treatment of fractures, it is used as a temporary fixation to control fracture displacement and pain at the present time. Its usage for definitive treatment is limited and often it is used for temporary treatment. Skeletal traction permits for more weight and longer time of traction and reduced skin problems in comparison with skin traction. Skeletal traction can be applied through femur, tibia and calcaneus in lower extremity. Femoral skeletal traction can be used for initial treatment of acetabular fractures, proximal femoral fractures and hip dislocations. Tibial skeletal traction is preferred for initial treatment of femoral shaft fractures and calcaneal skeletal traction is used for initial treatment of tibia and ankle fractures.
