Basic soft tissue procedures in hallux valgus surgery

The basic pathology in hallux valgus is soft tissue changes around the first metatarsophalangeal joint. Therefore, soft tissue interventions are important for a successful hallux valgus surgery. Lateral release and medial capsular plication are two basic procedures in soft tissue surgery. In order to perform lateral release correctly and effectively, it is necessary to know the pathophysiological changes in the soft tissue well. Lateral release and medial capsular plication aim to reduce the intermetatarsal angle, correct the hallux valgus deformity, and correct the metatarsosesamoid complex. While lateral release can be performed with single incision, double incision, transarticular release, percutaneous release, endoscopic and arthroscopic approach, medial capsular repair and plication can be performed with the incision used for metatarsal osteotomy. Many studies have been conducted especially on lateral release, and in these studies the necessity, effectiveness and possible complications of lateral release have been revealed. After lateral release surgery, complications such as recurrence or insufficient release, excessive release, avascular necrosis, and iatrogenic neurovascular and anatomical structure injury may occur. Our aim in this review is to describe the basic soft tissue interventions used in hallux valgus surgery, describe the frequently and currently used surgical techniques in the literature, and talk about possible complications.
