Radiologic features of soft tissue sarcomas

Soft tissue sarcomas are rare tumors of mesenchymal origin. Imaging methods are of great importance in the detection of sarcomas, local staging, determination of treatment, treatment response and evaluation of recurrences. In addition to patient age and clinical examination findings, the gold standard imaging method is magnetic resonance imaging (MRI). Thanks to the high soft tissue resolution in MRI, detailed anatomical localization, size, fat, hemorrhage, fibrous component as well as neurovascular invasion, presence of peritumoral edema and peritumoral enhancement, which are poor prognostic factors, can be detected in soft tissue sarcomas. The cellularity of the tumor can be evaluated with diffusion MRI and the vascularity, capillary permeability and interstitial space of the tumor can be evaluated with contrast-enhanced dynamic MRI. These data gain importance in the evaluation of treatment response, especially in deciding on tumor surgery. In this review, the classification of soft tissue sarcomas, imaging methods used in the evaluation will be explained in detail, especially MRI, and the findings in MRI in local staging, follow-up and treatment response will be revealed together with current developments.
