Sesamoid bone reduction and rotational correction

Hallux valgus is a multiplanar foot deformity and pre-operative evaluation should be performed carefully in order to achieve patient satisfaction and substantial surgical results. While it was previously thought to be a biplanar deformity, recent studies have shown that coronal plane alignment is also a component of hallux valgus. Since sesamoid bone position and first metatarsal pronation cannot be adequately evaluated on direct radiographs, weight bearing computed tomography should be the main method for surgical decision making. In addition, since intra-operative weight bearing radiographs cannot be taken, pre-operative radiological evaluation becomes even more important. Therefore, in hallux valgus surgery, standing computed tomography imaging should be performed to minimize the share of surgical error and to determine the appropriate surgical technique as recommended in the literature.
