Anterior cruciate ligament (ACL) reconstruction is the most common ligament injury surgery performed by orthopedic surgeons who are interested in sports injuries. Although many reconstruction methods have been described historically, controversy about the femoral and tibial tunnel locations, which determine the main success of surgery, is continuing. Intra-articular anatomical reference landmarks, which are used to determine the location of the tunnels, are described. However, the most important difficulties for orthopedic surgeons are; arthroscopic imaging and intraoperative availability of these landmarks. The aim of this review is to review the current studies in the literature about far anteromedial portal to establish femoral tunnel and reference landmarks, which are used for the anatomical location of the femoral and tibial tunnels, which are considered the most important factor in the success of ACL surgery.
