Patellofemoral joint arthrosis and arthroplasty

Patellofemoral arthroplasty is an effective treatment alternative for patellofemoral arthritis caused by primary osteoarthritis, dysplasia, or in patients younger than 55 or 60 years who have normal patellofemoral alignment without any subluxation. Patients who underwent distal realignment procedures previously, such as tibial tubercle anteromedialization or direct anteriorization may be candidates for patellofemoral arthroplasty. Patellofemoral arthroplasty is a true pain reliever in patients with isolated patellofemoral arthrosis, although the technique of the procedure is challenging and the outcomes are directly associated with the geometric features of the implant. Residual instability may result in early failure. Thus, patients with uncorrectable patellar instability or malalignment should be excluded. Implant malposition may also contribute to the failure of maltracking and mechanical catching of the patella. Preserved tibiofemoral compartments, both menisci, and cruciate ligaments may contribute to a more kinematic knee, compared to total arthroplasty. It is a particularly attractive option for young and active patients with isolated patellofemoral arthrosis in whom neither total knee arthroplasty nor patellectomy is desirable, and it can be easily converted to total knee arthroplasty, if necessary. With emerging designs for patellofemoral arthroplasty, the incidence of anterior knee pain following patellofemoral arthroplasty is comparable to that after total knee replacement surgery, namely, ranging between 4% to 7%. As even small amaounts of tibiofemoral cartilage loss may compromise the outcomes, the procedure should be restricted with the younger patients.
