Soft tissue injuries around the foot and ankle account for approximately 25% of all sports injuries in elite athletes. These injuries are particularly prevalent in sports such as football, volleyball, and basketball. Although these injuries can be classified according to their anatomical localization, combined injuries are often encountered in clinical practice. Therefore, understanding the characteristics of these injuries and being able to make a differential diagnosis allows for the rapid initiation of treatment for the athlete. In elite athletes, the goal is not only to return to play but also to restore the athlete to their previous performance level as quickly as possible. These injuries can generally be categorized under the following headings: ankle sprains, syndesmotic injuries, peroneal tendon injuries, tibialis anterior tendon pathologies, Achilles tendon pathologies, and posterior ankle impingement syndrome. It is crucial to keep in mind the physical examination findings and radiological features of these injuries. Treatment protocols should be carefully followed. In particular, post-operative rehabilitation is of paramount importance in cases requiring surgery, as it is the main factor in preparing the athlete for return to play. Therefore, a strong connection between the player, physician, and physiotherapist is essential.
