Intertrochanteric fractures are commonly seen fractures in elderly and osteoporotic patients and usually presents after low energy injuries. It is proven that there is a positive effect of surgical treatment on rehabilitation and functional recovery of these patients. The union of this type of fractures and functional recovery of the patients depend on both the patient related factors such as comorbidities, level of osteoporosis and also surgical factors as type of fracture, time of surgery after fracture, quality of reduction, type of the surgical method and implant used. There is a still debate about the type of surgery as internal fixation (with plate or proximal femoral nail) or arthroplasty in the treatment of intertrochanteric fractures which are extracapsular hip fractures. In this review, with the comparison of the meta analyses, we will compare the internal fixation and arthroplasty in terms of patients functional results, morbidity and mortality rates, time of hospital stay, bleeding and need of transfusion, need of resurgery, union time and complications.
