We present a case of delayed and persistent gluteal hematoma due to superior gluteal artery injury after an acetabular rim fracture reconstruction. To the best of our knowledge, with a different clinical picture from the previous reports. In this case report, we present a patient who was in a good clinical condition when admitted to the clinic for a wound that had developed due to a persistent gluteal hematoma. The patient was coincidentally found to have an unstable hemodynamic status by routine blood count and the correct diagnosis was made by a selective arterial angiography. The radiographically controlled arterial embolization of the lacerated vessel performed in this patient was demonstrated to be a quick and effective technique in the solution of this lifethreatening problem.
