Management of anterior pelvic injuries: surgical approaches and reduction and fixation techniques

Pelvic ring fracture is one of the commonly seen injuries with a higher rate of morbidity and mortality. The spectrum of the injury is highly variable, ranging from mild pain and discomfort to pre-hospital death. Successful management of unstable pelvic ring is a challenge for the orthopaedist. Accurate and early reduction with stable fixation minimizes morbidity and mortality rate and improves long-term outcome. In some cases, anterior fixation is required alone to restore a stable and symmetrical pelvic ring. However, combined anterior and posterior fixation of the pelvic ring is mostly required. An open or closed reduction technique can be performed. There are many factors influencing the choice of surgical procedure; however, the degree of instability of the anterior and posterior pelvic ring is the most determining factor.
